Required Documents

  • Completed application form
  • Certified copy of birth certificate
  • Most recent school report
  • ID copies of both parents/guardians
  • Proof of residence
  • Two passport-sized photographs

How to Apply

  1. Download the application form below
  2. Fill out all required information
  3. Gather required documents (birth certificate, ID copies, etc.)
  4. Submit the completed form and documents to our office
  5. Attend the interview (if required)
  6. Complete the registration process upon acceptance
